The information on this page may be useful if you have been referred for an individual placement.

What are placements?

Individual placements are needed for some people because they have very specific healthcare needs. The word placement does not necessarily mean that an individual needs to be physically placed somewhere. Some types of care can be delivered at home.

Individual placements cover:

  • Continuing healthcare and inpatient neuro rehab
  • Out of area specialist mental health treatment, including for people with a learning disability
  • Some specialist children’s and young people’s services
  • Provision of some specialist equipment


The NHS wants to provide high quality care and ensure you receive an excellent service. This information outlines the different stages of the process so you know what to expect at each step.

What happens?

If you are referred, the first part of the process is an assessment. This will decide if you are eligible for funding for an individual placement. Eligibility criteria for continuing healthcare placements are defined by NHS England . For other placements, local eligibility criteria are in place. They have been set the clinical commissioning groups for east and west Kent.

The assessment will vary depending on the type of care you have been referred for. It will take place at a time and location agreed with you, and you can have family and/or other representatives with you if you wish.

The results of the assessment will be reviewed. For mental health and children’s continuing care, this will involve a panel review, and will take place within a few days of the initial assessment.

After your assessment

You will be notified in writing of the outcome of your assessment. If you are not eligible, the reasons will be explained in the letter. Not being eligible for funding for an individual placement does not mean you are not eligible for care. It simply means that there are other services that can meet your needs.

You will also be given details of how to appeal if you feel the decision is wrong. The professional who referred you will also be informed of the outcome of your assessment.

Personal health budgets

If you are eligible for continuing healthcare, you will be offered a personal health budget. This will give you more control  over how to spend the funding to best meet your needs and achieve your health goals. It is your choice whether you want a personal health budget.  You will receive more information and details at this stage to help you make the decision.

If you are eligible for a personal health budget and choose this option, you will be helped to develop your healthcare support plan to be paid for by the budget allocated to you. This will allow you to control the nature and the timing of your care, and when you want it to start.

If you are not eligible for a personal health budget or choose not to have one, your healthcare needs will be managed on your behalf. The time this takes to implement can vary depending on the nature of the care, but you will be kept informed at all stages.


Reviews will take place to see if your needs are the same or have changed, and to check that the placement is meeting your needs. The placements officer will tell you when a review is due.

We want to make the process of providing the very best healthcare for your needs as straightforward and swift as possible while also making sure it is thorough and fair.

There are key points throughout the process where you will be asked for your feedback on your experiences. Everything you tell us will help us to provide the very best service for you and future patients.
